Our Process

Intial Assessment

Focusing on a short list of the most prevalent conditions captures greater than 80 percent of the unrealized value by hospitals and clinics as a result of inadequate documentation. Our team does an initial assessment of your chart data to determine the best opportunities for clinical documentation improvement. We then match the training content to the high-value opportunities found.

Educational Tools

To accommodate different learning styles and busy schedules, our solution includes a variety of learning tools that encourage participation.

  • Clinical Condition Cards and Sheets

    Whether high-tech or just plain practical, ModusOne helps physicians accurately document conditions. We provide pocket reference cards that can be used as a quick reference or study aid.

  • Sessions

    Using both in-person and online sessions, we initially train physicians on 10-20 of the most commonly missed high-value ICD-10 conditions. After physicians master the initial list, additional conditions and ICD-10 concepts can be added. Participants are able to learn standard definitions and become more effective at clinical documentation, no longer relying on clinical acumen alone.

    Our physician-led sessions follow best practices of adult-learning theory and give participants the opportunity to ask questions of experts in their field and to share experiences with others who are experiencing the same challenges.

    Online sessions are convenient and effective. Our participants typically achieve 80-100 percent on the session post-test. Our online training is based on the latest adult-learning techniques and emphasizes active and interactive methods to engage the learner. Individuals who attend sessions typically improve quality scores by 10-20 percent.

  • Mobile App with Participation and Score Tracking

    Your physicians can learn in short bursts of time more easily worked into their schedules. Our mobile app may be used on any computer, tablet or smart phone. The app quizzes the user on realistic clinical documentation scenarios. Each mini quiz is scored for effectiveness. Users can see their current score and compare to their best. Hospitals and providers that use the app achieve accuracy rates that are 10-20 percent higher than clients that rely on offline training alone.

Monthly WebEx Sessions to Review Progress

Our CDI solution includes a monthly progress review. Your clinicians will learn their quality scores and how they compare with past performance, along with strategies for continued improvement.

Monthly Chart Review and Documentation Accuracy

No clinical documentation improvement program would be complete without the metrics to show progress and provide accountability. We provide monthly progress reports that provide physician documentation scores based on chart data review. Typically within a year, the monthly chart reviews help organizations increase accuracy by 80-90 percent, on average.

We also provide monthly reports that include the metrics for the following:

  • Group accuracy rate
  • Individual provider accuracy rate
  • Detailed review of errors, including under- and over-documentation errors
